Player & Team Credentials Required

• Team Roster (since many clubs have not yet released 2023-24 seasonal rosters see below)
  • Roster can be from the 2022-2023 seasonal year 
  • Roster can be from the 2023-2024 seasonal year 
  • Roster can be typewritten (include player name, birth date, year of graduation) 
    • IF Roster is typewritten, all listed players must correspond to player cards as proof of registration 
  • Guest Players - can be handwritten at the bottom of the roster
    • Must have Medical Release form for each guest player 
    • Guest Players must have player card issued from 2022-23 or 2023-24 seasonal year (see below under "Player Pass Cards" for any player who has not received a pass card for the upcoming year and also does not have a passcard from the previous year). 
• Medical Release Form - each player must have a medical release form uploaded 
  • Teams are required to keep copies of the medical release forms on their person at all times 
• Player Pass Cards or Player Cards
  • Teams are requested to have physical or digital copies of player cards available to be checked field side. 
    • If a player does NOT have player cards/player passes for 2023-24 or 2022-23 then some form of player identity is required (drivers license, passport, school ID) AND documented "proof of registration" is required, including any of the options below: 
      • Name on a roster (from 2022-23 or 2023-24) * Upload this
      • Letter from a club registrar validating the player is/was registered between 2022-2023 or 2023-2024. ATTACH TO ROSTER. 
      • Letter from Club Director, or Team Manager or Team Coach validating player's registration in 2022-23 or 2023-24. ATTACH TO ROSTER.
